 Membership cannot be combined with any other offers. Program has no monetary value. Services may not be combined. Program services cannot be transferred. Membership cancellation prior to commitment period will be charged the equivalent of 3 months service fee. Monthly services DO NOT rollover month to month, however, the amount paid may be banked for future use on services. Other restrictions may apply – see staff member for details. NO show to monthly member appointment forfeits monthly service for current month. We reserve the right to cancel or modify memberships at any time. 24 hour appointment cancellation allowed for members only. Failure to cancel within 24 hours will result in forfeiture of the monthly service. Payments not received by the 5th of the month will incur a $25 late fee.
